If you are a celebrity, and would like to walk the Red Carpet at Cinco 4 at 10:00 AM on Sun, May 15th at Avalon Hollywood in LA, please fill out the form below. Space is limited, however we will do our best to accommodate as many red carpet requests as possible. 

Please note that by submitting this form you are REQUESTING Red Carpet Access.  This DOES NOT GRANT you access.  You will be contacted by a KLiK Representative if you are selected to walk the Red Carpet, and then you will receive credentials.
